EARLY CHILDHOOD BUILDING CAFETERIA
(112 N. FOURTH)

YOU MUST HAVE ALL DOCUMENTATION FOR SLOT TO BE HELD:
BIRTH CERTIFICATE (child must be 4 before September 1, 2026)
CURRENT IMMUNIZATION RECORD
SOCIAL SECURITY CARD
PROOF OF RESIDENCE (Current water, gas, electric utility bill or lease/rental agreement)
BOTH CDIB & TRIBAL MEMBERSHIP (if applicable)
MEDICAID CARD (if applicable)
PARENT’S PHOTO ID
PROOF OF INCOME (Pay stub or direct deposit) Pre-k students only.
